The Hospice Clinical Educator will work at a Visiting Physicians Association (VPA) office and serves as the clinical care liaison for U.S. Medical Management (USMM). The candidate will work closely with assigned physicians regarding criteria for hospice/home care referrals and agencies credentialed to use and protocols for lab tests and diagnostic tests, etc.

Duties to include:

• Provides on-site clinical coordination
• Determines quality community agency providers utilizing established criteria and benchmark standards
• Provides expectations to vendors regarding consistent communication regarding the patient's status, documentation, physician reports, etc
• Researches, organizes and develops a community resource manual
• Performs case management role as necessary
• Exhibits excellent service behavior to all physicians, other VPA staff, patients and their caregivers, community vendor agencies, and during all other encounters
• Demonstrates interpersonal skills that promote communication, collaboration, problem resolution, and team work
• Maintains communication with Corporate Director regarding compliance and in meeting job requirements
• Familiar with hospice/home health care referral criteria and services
• Working knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)
• Coordinates paperwork regarding C.P.O, certs and re-certs and Hospice/Home Health Care documentation
